NBC10-TV Spotlights Shreya Kangovi Health Navigators Work

Local Residents Trained to Assist Neighbors in Accessing Health Care Services

A West Philadelphia health care support program created by LDI Senior Fellow Shreya Kangovi is the subject of an NBC 10 TV feature story. The program -- the Patient-Centered Transition (PaCT) Project -- is a hospital-to-ambulatory care transition intervention that trains local neighborhood residents to be "navigators" or health assistants to their neighbors. They don't provide health care but rather educate and support low-income patients after discharge from the hospital. The program is also a study to see if the training and use of such local navigators impacts patient adherence and outcomes.
