Caring for Patients After Hospital Discharge during a Pandemic

One of the challenges in the current pandemic is how, and where, to properly care for COVID-19 patients once they leave the hospital. Many patients recovering from COVID-19 will need ongoing care to recover from hospitalization, but current models of post-acute in nursing homes, rehabilitation facilities, and home health agencies are ill-suited to care for them. How can we minimize the risk posed of transmission from post-acute patients to vulnerable long-term residents of these facilities? How can we support families in caring for these patients at home, given the inadequacy of the home health care workforce and the continuing mandate to distance socially? A panel of experts explored solutions to the post-acute care crisis posed by COVID-19.
