Karen Hirschman, PhD, MSW is the NewCourtland Term Chair in Health Transitions Research and a Research Professor in Penn’s School of Nursing. As a nationally recognized researcher, Dr. Hirschman’s career has focused on improving health care transitions for older adults and their family caregivers, with a specific emphasis on those living with cognitive impairment. She is a member of the Transitional Care Model team, led by Dr. Mary Naylor. This rigorously-tested advanced practice registered nurse-led model targets high-risk hospitalized older adults and focuses on transitional care to improve outcomes for older adults and their family caregivers. The team studies both replication and adaptation of the Model into health care settings and fidelity to implementation of the Model’s components. Dr. Hirschman is a Fellow of the Gerontological Society of America.
